Medical Release
In consideration of your accepting this registration, I the undersigned, intending to be legally bound, hereby, for myself, my heirs, executors and administrators, waive and release all rights and claims for damages I may have against CrossPoint, a Southern Baptist Church, The CrossPoint Foundation, the city of Springville or any other sponsors, promoters, persons or entities associated with this event or their agents, employees or otherwise. I attest that as a participant that I am physically fit and fully understand the risk of participating in athletic training and competition. Further, I grant full permission to any and all the foregoing to any photographs, videotapes, motion pictures, and any other records of this event for any legitimate purpose.
Covid Safety Acknowledgement
In light of the ongoing spread of COVID-19, individuals who fall within any of the categories below should not engage in the CrossPoint Sports Clinic and/or other face to face activities. By attending the CrossPoint Sports Clinic event, you certify that you do not fall into any of the following categories:
1. Individuals who currently or within the past fourteen (14) days have experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath or a loss of taste or smell; or
2. Individuals who believe that they may have been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for their treatment.
